About the testicles
The testes are two elongated glands and part of the
man's genitals. The testes are in the scrotum under the penis. They hang inside
the bottom of a connecteve tissue sack that is folded down from the belly bottom
into the scotum. The nerves and the blood vessels to and from the testicles and
the spermatic cord go iniside this connective tissue sack through the inguinal
channel just over the scrotum. This sack is called tunica vaginalis.
The testicles hang in a sack outside the body, so that they will cool down.
This lower temperature is necessary in humans to produce sperm and the male sex hormone testosterone.
The testes are connected with the body also with a thin muscle
going inside the tunica vaginalis that pulls the testicle more or less up, so
that the testicle allways is correctly warm.
The testicles hang loose inside the scrotum, and the
scotum has one room for each testicle. The outside of the testicles and inside
of the scrotum are very smooth and are also lubricated with a fluid. This way
the testicles are free to move inside the scrotum. This freedom of movement
provides that it is very difficult to hurt a testicle because it just slips away
when something is hitting.
The spermatic cord comes out from the testicles at the
upper back side as a lot of tubes that connect to this thicker tube. The main
blood artery to the testicle, the spermatic artery, goes into the testicle at
the backside some further down. The spermatic cord makes a loop downwards before
ascending up inside the teunica vaginalis and through the inguinal channel.
These structures at the backside of the testicle,
surrounding connective tissue, and the part of the surrounding part or the
tunica vaginalis just behind the testicle make an elongated structure called the
bitesticle or epididymis.
Around the birth the testicles are originally placed in
the abdominal cavity, but the sacks with the testicles fold down into the
scrotum early in infancy.
The testes
produce both sperm and male sex hormones. In childhood the testicles produce
some hormons, but do not produce sperm cells until puberty.
Unless you are left-handed, the left testicle hang lower down than the
right.
The structure of the testicle
In the inside of the testicles there are twisted, small
tubes called seminiferous tubules where the sperm cells are produced. Between
these tubes there are tissue that produce hormones.
The testicle has at its outside a connective tissue
sheet called tunica albuginea. Walls of connective tissue (septa) extend from
the tunica albuginea inwards in the testicle and divides it in compartments.
In each of the compartment the seminiferous tubules
assamble to a common tube that go out backwards of the compartment. At the
backside of each compartment these tubes assamble to a common tube innside the testicle.
This common tube again spread to form several tubes that
go out of the testicle at the upper backside part into the epididymis. And here
the tubes again run together to form the spermatic cord.
About the production of semen
Germ cells, spermatoblats, that are in the inner walls
of the seminiferous tubules produce spermcells by a kind of division,
so-called meiosis. By meiosis a normal divisjon first occur so that there
is produced a cell that resamble the spermatoblast. Then this new cell goes
through a special divison where the two cromosomes in each pair are separated so
that the new cells gets 23 chromosomes, and not the normal 46 chromosomes. These
new cells then gradually change shape so they become sperm cells.
The maturization of cells to sperm cells is a gradual
process, and as the new cells mature they are mooved towards the inner space of
the duct, while the spermatoblasts deeper in the inner layer of the duct
steadily divide to produce new cells by division.
In a man one of the chromosome pairs have two unlike
chromosomes, an X-chromosome and an Y-chromosome. A woman have two x-chromosomes
in this pair.
The meiosis provides that some sperm-cells have an
x-chromosome, and some have an Y-chromosome.
When spermcells combine with an eggcell in the womans
uterus, the combination will make a fertilized egg with two x-chromosomes if the
sperm-cell had an x-chromosome, and the baby will be a girl.
If the sperm-cell have an y-chromosome, the combination
will be a fertilized egg with an x-chromosome and an y-chromosome, and the baby will be a boy.
The testes are made of the same substance that is to the ovaries in
women.
About the production of male hormons
Leydig cells, cells localized between seminiferous
tubules produce the hormone testosterone and other androgens important for
sexual development during puberty and for maintaining the testicular maturity
and other mature characteristics throughout life.
The follicle stimulating hormone from the anterior
pituitary regulates, that is mostly stimulates, the production of testosterone.
A testicle and the near structures, like
the vas deference and the spermatic artery. The details at the backside together
with surrounding connective tissue is called the bi-testicle or epididymis.

About testosterone
Chemical properties and production
Like other steroid hormones, testosterone is derived from cholesterol. The largest amounts of testosterone are produced by the testes in men, but it is also synthesized in smaller quantities in women by the thecal cells of the ovaries, by the placenta, as well as by the zona reticularis of the adrenal cortex in both sexes.
The effects of testosterone before birth
Most of the prenatal androgen effects occur between 7 and 12 weeks of gestation. The stimulated processes in the fetus are:
- Formation of thes scrotal sack
- Genital virilization (midline fusion, phallic urethra, scrotal thinning and rugation, phallic enlargement); although the role of testosterone is far smaller than that of Dihydrotestosterone.
- Development of prostate and seminal vesicles
Without this stimulation, female-like organ can develope.
Early infancy androgen effects
Early infancy androgen effects are the least understood. In the first weeks of life for male infants, testosterone levels rise. The levels remain in a pubertal range for a few months, but usually reach the barely detectable levels of childhood by 4-6 months of age. The function of this rise in humans is unknown. It has been speculated that "brain masculinization" is occurring since no significant changes have been identified in other parts of the body.
Effects in early puberty - Effects in preteen and early teenage boys and girls
These are the first visible effects of rising androgen levels in childhood, and occur in both boys and girls in puberty.

Efects during later puberty
Advanced pubertal effects begin to occur when androgen has been higher than normal adult female levels for months or years. In males these occur usually in the later stages of puberty, and occur in women after prolonged periods of heightened levels of free testosterone in the blood.
- Phallic enlargement (including clitoromegaly)
- Increased libido and frequency of erection or clitoral engorgement
- Pubic hair extends and may reach thighs and up toward umbilicus, but here there are great individual and racial variations that do not depend on testosterone.
- Facial hair (sideburns, beard, moustache), but within great individual and racial variations that are not tied to testosterone.
- Chest hair, periareolar hair, perianal hair
- Subcutaneous fat in face decreases
- Increased muscle strength and mass
- Deepening of voice
- Growth of the adam's apple
- Growth of spermatogenic tissue in testes, male fertility
- Growth of jaw, brow, chin, nose, and remodeling of facial bone contours
- Shoulders widen and rib cage expands
- Completion of bone maturation and termination of growth. This occurs indirectly via estradiol metabolites and hence more gradually in men than women.
The extent of body hair coverage vary considerably between individuals and between different races.Some races have much body hair, other very little.
Adult testosterone effects
Adult testosterone effects are more clearly demonstrable in males than in females, but are likely important to both sexes. Some of these effects may decline as testosterone levels decline in the later decades of adult life.

Excessive testosterone in males was believed to lead to an increased risk of prostate cancer. Recent studies suggest that this is not thrue, but excsessive testosterone levels may increase the rate of spread of an allready existing prostate cancer, The levels of oestrogen has probably more of a role in causing prostate cancer.
The testosterone is metabolized in the body to a variant called dihydroy-testosterone. This variant have many of the same effects as testosterone and the effects of this variant is even stronger that that of testosterone in some ways. Dihydroxy-testosterone inhibits the growth of hair on the scalp. Dihydroxy-testosterone stimulates the prostate to grow and is responsible for much of the benign prostate hyperplacia that many men get as they age.
The male hormonal regulation
The production of testosterone is itself regulated by hormones from the anterior pituitary glands, thes follicle stimulating hormone (FSH). This hormone is found also in females, and by them stimulate female genital processes.
Lack of testosterone and consequences of this conditions
The production of testosterone usually declines with age. Also diseases in the testicles or injury to the testicles can cause decrease in the testosterone levels. Too low production of FSH in the pituitary or hypothalamic disturbances can further cause a decreased production of testosterone. Some boys never get a full testosterone production during puberty and will suffer form problems due to this. Lack of testosterone causes several symptoms:
- The muscle mass declines.
- The body accumulates more fat, and likely at the bad sites, like around the mid.
- The testicles will shrink in size.
- Psychological symptoms, like axiety can develope. Meny men will also think and react slower.
- There will be a decreases interest for sex and decreased sexual arousal.
- There will be problems with achieving good erections.
- There will be less bone desity.
- The baird will grow less well.
- The pubic hair will be thinner.
As somewhat good effect of low testosterone levels will be less hair loss on the scalp, and less tendensy to baldness.
The effects of testosterone supplements
Supplying testosterone to the body can reverse many of the symptoms associated with low testosterone levels. This can be done with injections, with tablets to chew so that the bormone is absorbed through the buccal nucosa, or with transdermal patches.
However by supplying the hormone itself, the body looses its controle of the testosterone levels. It can easily get too high at times and still to low at other times.This lack of controle can give adverse effects, for example breast development, increased tendensy for prostate enlargement, increased chance of developing prostate cancer, water retention and edema, heart problems and too prolonged erections that will not go down.
A better way of remending low testosterne levels is therefore to supply the body with stimulants that enhances the body's abilioty to produce testosterone and at the same time increases the ability to controle the testosterone production.
