About inflammatory diseases in the intestines

There are many variants of diseases in the intestines that give inflammation as the main manifestation. The causes of these diseases can be an autoimmune reaction, infection or irritations by substances. Two fairly typical variants are Chron's disease and ulcerative colitis. Sometimes it is not possible to give a more specific diagnosis than an unclassified inflammatory disease in one or more parts of the digestive system.

What is Chron's disease

By Chrons's disease there is an inflammation in the small intestine, the Colon, both the colon and the small intestine and sometimes other digestive organs. The inflammation is located in all the levels of the intestinal wall. This inflammation disturbes the intestinal function and the uptake of nutrients. The inflammation can also make sores leading to bleading and scar formation.

Crohn's ileitis is the disease attacking the terminal part of the small intestine constitutes thirty percent of cases.

Ileocolic Crohn's disease, which attacks both the ileum and the large intestine, gives fifty percent of occurances.

Crohn's colitis, affecting the large intestine, constitute the rest twenty percent of cases and this variant may be particularly difficult to distinguish from ulcerative colitis.

The disease tend to attack sections of the intestines and leave the sections in between unattacked.

The disease can attack other parts of the body like the eyes, joits and skin.

Seronegative spondyloarthropathy can be a part of the total syndrome attacking the patient which is an inflammation in the joints or the vertebral column.

There can be inflammation of one or more joints (arthritis) and inflamed ares in the muscles (enthesitis). The arthritis can affect larger joints such as the knee or shoulder or may exclusively involve the small joints of the hand and feet.

The arthritis may also involve the spine, leading to ankylosing spondylitis if the entire spine is involved or simply sacroiliitis if only the lower spine is involved.

The symptoms of Chron's disease

Symptoms of Chron's disease vary according to the severity of  the disease.  The condition is chronic , but with periodic flare-ups and remissions. The symptoms use to come gradually and develop often through several years.

Attack of the disease in the mouth can lead to sores that do not heal easily.

When arthritis accompany chron's disease, there are  painful, warm, swollen, stiff joints, mobile joints and decreased joint function. When the spine is attacked, stiffened spine can be the result.

The causes and process of Chron's disease

The causes of the disease are not well known. There may be an inherited factor. There seem to be a auto-immune reaction against the bodie's own tissue. The inflammation makes it difficult for the intestines to absorbe nutrients.

The inflammation uses to span the whole thickness of the intestine affected. This is different from ulcerative Colitis that mostly affects the mucosa or inner layers.

The inflamed intestines are more susceotible for infections by bacteria so that affected sections are often infected, but infection is not the primary mechanism of the disease.

The inflammation can make uptake of nutrient less efficient. The bleedings will furter deplete the body from nutrients. When healing, there can form scars, and the scar formations can restrict the passage for intestinal content through the intestines.

Traditional treatment of Chron's disease

The inflammation process are usually treated by anti-inflammatory drugs, like steroids.

Antibiotics are used aginst complicating infections and sometimes against the basic disease process.

The treatment further aims to correct deficiencies of nutrients caused by the disease.

By uncontrollable bleeding and extensive scar formation, surgery is uften used.

About ulcerative colitis

By ulcerative colitis there is an inflammation mainly in the Colon, but the inflammation can to some extend also affect many other organs. The inflammation is located mainly in the mucosa and inner layers of the intestinal wall. This is a main difference from Chron's disease. The inflammation produces sores and scarring. Bleeding is a more prominent symptom than by Chron's disease.

The causes and process by ulcerative colitis

The exact causes of the disease are not known. There seems to be a genetical susceptibility to the disease by those suffering from it, since the disease tends to aggregate in families and geografical areas and since an identical twin has 10% chance of gettig it if the other twin has it.

The diseae is effected by an attack of the body's own immune system on the inner colon wall. The attack causes sores and ruptured blood vessels and thereby bleeding. The sores also cause body fluid leaking into the colon. The bleeding and leaking will over time cause anemia and depletion of nutrients and thereby general weakness that can be serious.

The inflammation will often attack other body areas, for example the joints, but usually in a mild degree.

The symptoms of ulcerative colitis

Symptoms of ulcerative colitis vary according to the severity of  the disease.  The condition is chronic , but with periodic flare-ups and remissions. The symptoms comes gradually more quickly than by Chron's disease.

Abdominal pain is a common symptom, but it can vary from mild discomfort to sharp pain.

A main symptom is diarrhoea muxed with slime.  Another main symptom is bleeding from the colon wall. The blood usually mixes with diarrhoea.

Mild symptoms of pain from the joints, for example the knee, is common and will often preceed the intestinal symptoms.

Traditional treatment of ulcerative colitis

By traditional therapy one has usually first a period of intensive treatment to effect remission and then a period with less intensive therapy to maintain freedom from symptoms.

Derivates of salicylic acid is often used when the symptoms are mild. The traditional preprate used is sulfasalazine. Other variants of this group are:  - Mesalazine, also known as 5-aminosalicylic acid, mesalamine, with brand names like Asacol, Pentasa, Mezavant, Lialda, and Salofalk. - Sulfasalazine, also known as Azulfidine. Balsalazide, also known as Colazal or Colazide (UK). - Olsalazine, also known as Dipentum.

By more serious symptoms corticosteroids or derivatives of corticosteroids are used, like: Cortisone, Prednisone , Prednisolone, Cortifoam, Hydrocortisone, Methylprednisolone, Beclometasone , Budesonide - under the brand name of Entocort.

Also immune suppressive drugs are used, like: - Mercaptopurine, also known as 6-Mercaptopurine, 6-MP and Purinethol. - Azathioprine, also known as Imuran, Azasan or Azamun, which metabolises to 6-MP.  - Methotrexate, which inhibits folic acid. - Tacrolimus.

By accute flear-ups with extensive bleeding and loss of dissolved electrolytes and other substances, blood transfusion and trasfusion of the lost components is necessary.

For severe disease not treatable by other means, surgical removal of the colon is a common treatment.

Alternative and new treatments of inflammations in the intestines

Chron's disease can possibly be treated by introducing hookworms into the intestines, and this treatment method isbeing tried out.

In China acupuncture is a standard treatment against Chron's diseae. It seem to work well against the symptoms and to some extend against the basic mechanisms of the disease.

Thalidomide and cannabis has shown to help against the inflammations by chron's disease, but these treatment methods imply legal problems and other health risks.

Omega-3-fatty acids have proven to help reduce inflammations, including inflammations in the digestive system. It has been tested clinically against Chron's disease and ulcerative cholitis and a decrease of inflammation markers in the blood was found. The efective substances seem to be eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA).

Extra supplements of vitamins, minerals and other essential nutrient may help to overcome the weakness caused by this disease and help the body repair damages caused by the disease process.

Uncooked fibers of brassica seems to stimulate healing of ulcers in the intestines and thereby help against these concequences of inflammatory digestive diseases.
