About scoliosis - causes, symptoms, diagnosis and treatment
Scoliosis is a deformations in the spinal column that usually progresses from small deviations and get gradually worse. The extent of the deformations often stops at a moderate stage, but can in some cases get very severe.
The deformation can be sidewise, rotational, foreward or backward, often combined. Often there are deformations at several places in the column in opposite directions so that the severity of the disease is not always discovered before some time. Sideways bendings in a S-shape is typical.
Ideopathic scoliosis is the most common type, and of which one does not know very well the original causes. Ideopathic scoliosis starts some time in childhood, most typically at the pretpubertal stage and pogresses throughout puberty until the growth of the spine is finished, but can get more severe also later. The deformations develop due to unsymmetrical growth in the spinal column, but without any degenarative disease in the spinal structure.
Even though the causes of this kind of scoliosis is not well known, one knows that the tendency to develop scoliosis is to some extent inherited. Chiropractors and cranioscral therapists have a hypothesis that ideopathic scoliosis is caused by blockings that hinder the circulation of cerebrospinal fluid within the column, the blood flow alomg the column and the functions of the nerves along the colum..These blockings are often thought to be caused by stiffness or scars in the muscular layers, in the tendons and fascial layers around the column, in the meningeal membranes around the spinal cord within the column, or of misalignment in the position of the spines according to each other.
Scoliosis can also be caused by neurological and muscular paralyses or malfunctions, so that the muscles excert a different force on one side than another. Also inflammational or degenerative processes in the spines or cartilages between the spines can cause scoliosis.
The main symptoms of scoliosis is of cource the bendings and twistings in the spinal colomn, but other symptoms also appear that one often recognizes before one sees that the spinal colomn has got an abnormal shape. These symptomns are: Leaning towards ine side, one hip higher than the other. better developed muscles on one side than the other, deformities in the breast cage and unsymmetrical waist. Later one can get back pain.
The diagosis of scoliosis is fairly simple. One first checks if on shoulder is higher than the other. Then one cheks if on hip crest is higher than the other. One looks and feels along the spine while the patient is standig upright to see the curvature. Often the bendings can be seen right away. To be more sure one also lets the patient bend foreward. Where the spine is curved towards one side, one will see a bump on that side of the back, because the bent part of the spine will be pressed outward on that side. By letting the patient do other bends and twistes with the torso, arms and legs, a more complete picture can be made.
During this test the patient must be naked on the back and also on the upper parts of the buttocks. The very best for the diagnosis is a totally naked patient, so that on can also see misalignment of the buttocks that are assosiated with scoliosis, but one usually lets the patient wear underpants and pulls these down more or less at some point in the test.
It is generally recommended that children and teens are examined by such a simple test each year. In many societies this is done in school or at wellchild exams. But parents should do the test on their children themselves to detect a beginning scoliosis as early as possible, at least two times a year.
If scoliosis is detected with such simple tests, and it looks severe enough, one will often do more thorrough examinations, especially with x-ray imaging. If the scoliosis is minimal, one usually will only follow the development to see if it is developing further and how fast it is progressing.
With somewhat more severe deformations the standard treatments are exercises and braces. By scoliosis starting very early, one sometimes uses casting to guide the growth in a right direction. Cranisacral therapists and chiropractors treat scoliosis by manipulations. The manipulations have the aim of taking away mechanical blocks thought to exists around and within the column and thereby reestabishing a healthy growth.
With a more severe development one often performs surgery, by which the cartilages between the spines are removed, the column is stretched out, metallic bars are fastened to the treated sections and bone grafts are placed between the spines so that they grow togeather. The metallic bars will usually remain in place permanently, but can be removed later on if they cause trouble.
By surgery at a very young age, one tend to use bars that can be stretched further out throughout the growth without laying grafts that make the spine stiff.
Surgery imply many hazards for complications both during surgery and during later development, and small or great sections of the spine gets stiff. Often late complications lead to new surgery. A common complication is neurological injuries amd even paralyses. Surgery for scoliosis is therefore not something one shall decide for after just an easy concideration.
Orthopedic Examinations in Children and Adults - How They are Done
An orthopedic examination is an integral part of a well-child periodic assessment, and it is also performed both on children and on adults upon special concerns. The extent of the examination varies however much from setting to setting.
By the orthopedic examination one usually begins by checking the gait. One lets the patiant go from and to the examinator while watching himher walking in both directions.

In addition to the back and scoliosis check, the checking of limbs, joints and their function is a main topic by the orthopedic examination, and especially at the legs and the pelvic zone. Usually the patient will wear short briefs only during this examination. In certain settings one prefere to examine children totally naked.
The shoulder and elbow joints will typically be tested in the next stage. Typically the examination of the upper extremities are performed like this:
- One feels along the arms of the patient from shoulders to fingers to check muscle tone, consistancy and temperature.
- On lets the patient swing the arms from a position pointing downwards to a position 90 degrees to the body and pointing foreward.
- One instructs the patient to bend the hands up and down, to each side, and to rotate the hands in both directions.
- On lets the patient swing the arms further so they are pointing stright upwards.
- The patient is then instructed to swing the arms down to a 90 degree to the body and pointing out to each side.
- Then the patient is asked to flex the elows to a 90 degree angle and swing the lower arms and hands in a circle in both directions.
Later on one usually checks the hip joints and the legs. The patient will usually be laying on the back on an examination table. One will then do several handlings and bendings of the legs to check range of movement:
- One feels along each legs to test consitancy and tone of the muscles and the temperature in the leg.
- With both leg streight out, one swings the foot outwards and inwards, up and down.
- The patient lays with straight legs and the examinator spreads the legs as far as possible out to each side.
- One flexes the knees of the patient up in a frog fashion and let hisher soles touch each other. Then the legs are spread down to each side.
- One lets the legs lay straight out on the examination table. With one leg still laying straight, one swings the other straight leg up as far as possible and then down again, and the foot is held so that the calves are maximally stretched.
- While one legs lays straight, one flexes the other leg up and tries to press the knee of that leg as far down towards the chest area as possible, while also trying to press the foot as far as possible down towards the thigh, and the foot is held so that the toes point upwards.
- Whith one leg laying straight, one flexes the other leg up so that the thigh and calve make a 90 degree angle, and then one swings the calve as far as possible inwards and outwards.
- With one leg laying straight, one holds under the foot and over the knee, lets the knee joint be flexed and rotate the leg around.
The same tests are then done with the other leg while the first leg is held straight. Many of these tests are often done in one continuous movement.
As said one will often prefere to have the child completely naked during these tests. This has the advantage that one can see if the cleft between the buttocks, the natal cleft, and the vulvar cleft in girls, are alligned normally. But the child might, depending much upon its cultural heritage, feel it to be embarressing being totally naked during these hip and leg tests.
Even though these tests are necessary for a thorrough test of the joints of the lower extremities, the very nature of the movements will also give a mechanical stimulation upon the genital organs and upon the area around the anus. The examinator possibly also looks for the nature of the reactions caused by this stimulation, but will never mention that heshe so does.
Even though it is never said openly, one possibly also prefere the child to be naked to test hisher psychological reactions upon being inspected rather intimately this way, or to see if the movements cause leackage from the urinary tract, the vagina or anus.
The pediatric services in most countries have a standing order form the authorities of being on constant allert for signs of sexual abuse of children or for signs that the children themselves have unwanted sexual habits, and suspect every parent of being abusers. Even though it is never said openly, it seems that the reactions of the child during these tests are used to assess the possibility of the child being a victim of abuse or of "self-abuse"
Orthopedic examination has traditionally been done manually in combination with active testing of the mobility and strenght of the patient, as described above. There is however a growing practice of using ultrasound sonds to inspect the joints in as standard othopedic check-up, and especially the hip-joints, shoulder joints, knees and elbows. .

Body-building has two main effects: You get bigger muscles. You get stronger, and your body changes shape. If you have not set any goal for the development of your bodily proportions, and planned your training according to these goals, your body-building activities can give results you do not like.
DEFINE YOUR GOAL FOR BODY SHAPE
First of all you must define goals for the proportions between your body parts. Many people will for this goal choose a classical standard like this: Slim around the mid, flat stomach, round buttocks, muscular thies and calves, strong muscles around the shoulders and on the breast, and fairly strong arms. This classical ideal gives a balance between the lower body and upper body.
Others, however, will prefere a more v-shaped body shape with nore muscle mass at the upper body than the lower.
You must also decide how much fat you want to have on your body. Some persons prefere to hae so little fat that the muscles are clearly visible, others prefere to have an even layer of fat that makes the body look smooth. Neither too little nor too much fat is hoever healthy.

Then you must compare your goal with your actual body shape, and note all points that need special attention in order to achieve these goals.
- You must define which body parts have to develop greater muscle mass
- You must define which body parts need to loose fat, or perhaps gain more fat.
- You must also decide if you want changes in you posture, or correct unwanted curvatures or bendings, and you must define these changes. By consistent training you can greatly improve your posture.
Then you should consider wheather your goals are realistic, and adjust your goals if they are not so.

METHODS TO USE FOR ACHIEVING YOUR GOAL
There are three main things defining your body shape: The amount and distribution of muscle mass, the amount and distribution of fat and the flexibility of your spinal culumn and your joints. In addition your digestive condition and general health condition can be of importance. A bad digestion can make your stomach engorged and this can have a very string impact upon your body shape. Many chronic conditions can cause edema, and also this can disturbe the body shape.
Therefore you must use several different methods to achieve the wanted shape: Muscle training, condition training, right diet, possibly slimming and streching exercises. Improvement of posture will result from the combination of stronger muscles and flexibility training.
If there are special condition causing engorgement of your stomach or edema, also these must be adressed. The exercises will to some extent also help against these problems, but in addition it may be necessary to address them directly.
YOUR BODY BUILDING PLAN
Then you must make a plan for your training, your slimming, your diet and
otherwise for your lifestyle. A repetitive three day's plan with training 45
minutes to one hour each day will give you great results, if the plan is
consistently adhered to:
In day one you perform exercises to gain muscular mass and muscular strength. You should train all your muscles, but those body parts where you have planned to get must muscles, should be exercised the most.
In day two you perform condition training. In day three you stretch out and get rest from hard exercises.
Weight lifting or analogous training where you work against reistance will develop your muscle strength and muscle mass.
To achieve the right shape, you must train the most the muscles in those body parts where you want to gain muscle mass.
The muscles in body parts where your allready have the prefered muscle volume shall get enough training to maintain your muscles and strength, but not more.
If you have posture problems, curvatures or bendings in body parts caused by weak muscles or bad habits, these can often be helped for by strengthening the muscles that flex the body parts in the opposite direction. If however, the bending is due to scoliosis or some other disease under progression, the problem need professional treatment.
You must also do exercises that enhance your general condition, like jogging, swimming, playing ball, and cycling. Condition training will improve your general health and make it easier for you to loose fat.
Choose something that you find funny, and you can also vary between different activities. It is also good to invite some friends to exercise together with you on this day.
If parts of your body are unelastic and little mobile, this wil cause parts of your body to curve in wrong directions. The most common is a foreward curvature of your upper spine and neck. To get a right body shape, you must correct this immobility and curvature by stretching exercises.
It is especially important to stretch out in the opposite directions of those where you have unwanted curvatures. Such exercises must be combined with exercises to strengthen the muscles that counteract the unwanted curvatures.
Yoga exercises are very good to enhance your flexibility and improve your general health. It is also advisable to set out some time for meditation the same day that you perform stretching exercises.
Slimming away unwanted fat is an important component of body sculpting. This does not necessarily mean that your weight goes down, since the increase of muscle size often will outweight the reduction of fat mass.
When you slim away unwanted fat, the fat in all your body parts will lessen. However, it is difficult to decide where the trimming effect will be greatest. The best advice is to slim down until no body parts has more fat then you want. Body parts that seem too slender after this process, can then be built out by muscle training.
To take away fat, you simply reduce your intake of carbohydrates and fat, but you must still get enough of carbohydrates and fat so that you have energy for your daily life and your muscular training.
HOW TO GET A SIX-PACK
To get a 6-pack you simply reduce your intake of carbohydrates and fat so that the layer of fat at your belly gets very thin, and at the same time you perform intensive training of your abdominal muscles. But take care that you do not reduce the fat and carbohydrate intake too much, however. Without an energy supply the muscular training does not work.
The diet is important for achieving and maintaining a good body shape. You must
eat 3-5 good meals each day, but do not over-eat.
You need enough proteins. Every meal must have some protein-rich food sources, like: Fish, seafood, lean meat, lean cheese, foul or mushrooms. Also nuts, almonds and sunflower seeds contain protein. These are good food sources, but because of the content of fat, you should not consume too much of them. Eggs are a good protein source, but eat it in moderate amounts to avoid getting too much cholesterol.
In each meal you need some fat from natural sources and you need some carbohydrates. You should however control your consume of fat and sugar. You must not add very much fat or sugar to your food, and you must not eat too much of food with a high content of fat and sugar.
Also vary between different natural fat sources to get all the essential fatty acids you need: Fat fish, nuts, sunflower seeds, almonds, olive oil, rape oil, canola oil, flaxseed oil, marine oils, nut oils and sunflower oil. Do not use only soy oil or corn oil as many people do. Neither use much butter. Do not consume chemically altered fat.
As sources of carbohydrates use mostly full corn bread or cereals, peas, lenses or beans, but also add some sweet fruit.
In order to get enough fiber, vitamins, minerals and anti-oxidants, every meal must have some fruit or vegetables in a natural condition. Supplements of vitamins, minerals and anti-oxidants and special nutrients can be of help.
